I hereby declare that I will not hold the company liable in case any accident happens resulting to any physical injury that I may incur during the session I workout in the premises.

I affirm that I have entered into this agreement freely and voluntarily.

I agree that I shall not be refunded of any payment I have made for enrollment. Any unused sessions in my subscription my be used on a different day and shall be used within a reasonable period.

I shall conform to the organization's rules and regulations, including but not limited to the use and care of facilities in the premises.

Weekly Pass
